Job summary

Hospital Valencia seeks an experienced Manager, Case Management to provide 24/7 leadership across case management, social work, utilization management, and discharge planning.

The role reports to the Director of Case Management and Social Work, partnering to drive clinical, psychosocial, and financial outcomes while leading a team through training, supervision, and quality improvement initiatives.

Qualifications

• Bachelor's degree in Nursing is required.
• Master's degree preferred.
• Current BLS Provider Care certification.
• National certification (ANCC, ACMA, or CMSA) preferred.
• Proven leadership in acute case management with cross-functional teams.

Responsibilities

• Provide leadership and oversight of case management, social work, UM, and discharge planning functions.
• Support program planning, workflow monitoring, and regulatory compliance.
• Train, supervise, and evaluate team members while fostering strong customer service and quality outcomes.

Job description

We are working with Hospital Valencia, CA who is seeking an experienced Manager, Case Management to provide 24/7 leadership across case management, social work, utilization management, and discharge planning. Reporting to the Director of Case Management and Social Work, you'll partner closely with the Director to drive clinical, psychosocial, and financial outcomes for patients and families — while leading a team through training, supervision, performance evaluation, and continuous quality improvement.

This is a high-visibility role, representing this facility to physicians, health plans, third-party payors, and administration alike.

What You’ll Do

• Provide leadership and oversight of case management, social work, UM, and discharge planning functions
• Support program planning, workflow monitoring, and regulatory compliance
• Train, supervise, and evaluate team members while fostering strong customer service and quality outcomes
• Serve as a key liaison between facility and internal/external stakeholders

What You Bring

Licensure & Certification

• Current BLS Provider Care
• National certification (ANCC, ACMA, or CMSA) preferred

Education

• Bachelor's degree in Nursing required
• Master's degree preferred

Experience

• Proven leadership in acute case management
• Experience with utilization management and discharge planning
• Track record leading innovative case management programs
• Skilled in clinical assessment for patients with complex medical, emotional, and social needs
• Team leadership and resource planning experience preferred

Knowledge & Skills

• Strong grasp of regulatory standards (CMS, Title 22, CHA Consent Manual, TJC) and professional guidelines (ACMA, CMSA, NASW)
• Excellent communication, collaboration, and negotiation skills
• Working knowledge of Meditech and InterQual Criteria
• Demonstrated success driving continuous quality improvement and benchmarking initiatives
