Job summary

TEEMA Group is seeking an experienced Caregiver 2 to provide high-quality personal care in a center-based setting in Fremont, CA. The role supports seniors and individuals with complex needs, mentoring Caregiver 1 staff and helping onboard new teammates.

A CNA or HHA certification with CPR and First Aid is required. The position emphasizes compassionate, patient-centered care, collaboration with a skilled team, and adherence to safety and documentation standards in a fast-paced clinical

Qualifications

• Minimum of one year experience with elderly or frail population.
• Experience with patients with dementia preferred or required for certain assignments.
• Active CNA or HHA certification is required.

Responsibilities

• Provide personal care and escort assistance per patient care plan.
• Assist with activities of daily living including grooming, dressing, meal prep, bathing, and hygiene.
• Observe changes in condition and report promptly to the team.
• Onboard and train Caregiver 1 and 2 team members using manuals and training materials.
• Demonstrate safe lifting techniques and coach others on proper procedures.
• Participate in departmental and family meetings as needed.
• Maintain infection control, safety practices, and professional conduct.
• Document care activities in forms or electronic health records.
• Work independently while coordinating with the healthcare team.
