Qualifications

• Minimum of one (1) year of experience providing personal care to frail or elderly population.
• Must possess a valid CA Food Handler’s Certificate.
• Experience navigating and documenting within EMR systems.

Responsibilities

• Provide expert assistance with activities of daily living (ADLs).
• Measure and accurately document vital signs.
• Assist with safe transfers and medical escort duties.
• Coordinate meal services and dietary plans.
• Facilitate participant activities with the Engagement team.
• Maintain cleanliness in participant areas.
• Complete all required clinical documentation using EMR.
