Job summary

TEEMA Group is seeking an experienced Caregiver 2 to provide high-quality personal care to older adults in a center-based healthcare setting. Under supervision, you will guide and support Caregiver 1 and 2 teammates, helping onboard and train staff while delivering compassionate, patient-centered care.

The role requires at least one year with elderly or frail populations, CNA or HHA certification, CPR, and First Aid.

Qualifications

• Minimum of one year of experience working with an elderly or frail population.
• Experience working with seniors with dementia.
• Certified Nursing Assistant (CNA) OR Home Health Aide (HHA) certification
• CPR certification
• First Aid certification

Responsibilities

• Provide personal care and escort assistance per each patient's individualized care plan.
• Assist with activities of daily living including grooming, dressing, peri care, showering, bathing, toileting, meal preparation, feeding, housekeeping, and laundry.
• Observe patients for changes in condition and communicate changes to the team.
• Provide onboarding and ongoing training to Caregiver 1 and 2 team members using established materials.
• Demonstrate safe use of lifting equipment and coach others on proper techniques.
• Participate in departmental and family meetings as needed.
• Maintain accurate care documentation in the electronic health record.
• Use basic computer applications including email and EHR.
• Work independently while collaborating with the healthcare team.
• Maintain a professional, respectful care environment.
• Perform other related duties as assigned.
