Job summary

Your Health in Rock Hill, SC is seeking a Senior Solutions Advisor to help patients navigate aging, chronic illness, and care transitions across facilities, homes, clinics, and virtual visits. You will assess needs, provide living arrangements guidance, and connect families with Your Health services and community resources.

As the regional point of contact, you’ll educate professionals, coordinate with care teams, and support market expansion through outreach and referrals.

Qualifications

• Bachelor’s Degree in Social Work, Human Services, Healthcare Administration, or related field preferred.
• Strong interpersonal and communication skills with families and providers.
• Must be a licensed driver with reliable transportation.

Responsibilities

• Meet with patients and families to assess long-term care needs and social support challenges.
• Provide consultative guidance on living arrangements, safety concerns, and available Your Health services.
• Serve as the regional point of contact for onboarding new patients.
• Maintain knowledge of local senior care resources and services.
• Act as liaison between patients, families and resources for coordination.
• Educate community professionals and caregivers through outreach and training.
• Timely communication with new patients in assigned Care Group.
• Support care transitions with Transitional Care Providers and care teams.
• Assist families in accessing care management and hospice services.
• Document activities for billing structures (CCM, ACP).
• Participate in community events and health fairs to promote SSA services.
• Collaborate to expand market through referrals.
