Apploi is seeking a compassionate Social Worker (LSW/CSW) for a part-time role at a skilled nursing facility in Lewes, Delaware. You will perform psychosocial assessments, assist with care planning, and support residents and families through transitions.

Qualifications

• State-certified LSW or CSW credential is required.

Responsibilities
