Mission Point Healthcare Services in Michigan seeks a Social Service Manager to provide medically related social work services so that residents may attain the highest practicable level of well-being. The role assesses emotional and behavioral problems related to illness and participates as a member of the interdisciplinary team.

Formación

• Requires a bachelor’s degree in social work and Michigan social worker license.
• Minimum one year of elderly social service experience or related field.
• Must be able to document social histories and care plans clearly.
• Strong communication with residents, families, and community resources.

Responsabilidades

• Assess psychosocial needs and develop goals for providing the necessary service and take part in admission process as needed.
• Attend care planning conferences and integrate social service goals into the resident's plan of care.
• Supervise social work interns and assist in staff development.
• Lead discharge planning and advocate for resident rights during IDT meetings.
• Coordinate services with psychiatric providers and community resources.
