Job summary

Adept of Omaha, located in Omaha, Nebraska, seeks a compassionate Social Services Director to lead the Social Services Department and support residents’ emotional, social, and psychosocial well-being.

You will work with residents, families, nursing, administration, therapy, and the interdisciplinary team to identify needs, coordinate services, assist with care planning, and support safe transitions and discharges while upholding resident rights and dignity.

Qualifications

• Experience in long-term care or related setting preferred.
• Knowledge of resident rights and care/discharge planning preferred.
• Strong communication and interpersonal skills.
• Experience with EMRs and long-term care documentation preferred.
• Professionalism, confidentiality, good judgment, resident-centered care.

Responsibilities

• Identify psychosocial needs of residents.
• Participate in care plan meetings and interdisciplinary conferences.
• Complete social services assessments and notes timely.
• Assist with admissions, transfers, discharges.
• Develop and coordinate discharge plans with residents, families and team.
• Maintain Social Services Dept records and HIPAA compliance.
• Educate staff on resident rights and psychosocial topics.
