Job summary

Qualifications

  • Experience in long-term care, skilled nursing, healthcare, social services, case management, or a related setting preferred.

Responsibilities

  • Identify and address the psychosocial needs of residents.
  • Participate in resident care plan meetings and interdisciplinary team conferences.
  • Complete required social services assessments, documentation, progress notes, and other records accurately and timely.
  • Assist with resident admissions, transfers, room changes, and discharges.
  • Develop and coordinate comprehensive discharge plans in collaboration with residents, families, the interdisciplinary team, healthcare providers, and community agencies.
  • Ensure appropriate follow-up is completed throughout the discharge-planning process to promote safe and successful resident transitions.
  • Communicate with residents and families regarding care planning, discharge planning, concerns, available services, and community resources.
  • Serve as an advocate for residents and help ensure resident rights, preferences, dignity, privacy, and individual choices are respected.
  • Mediate concerns involving residents, families, and staff in a professional and respectful manner.
  • Provide residents with emotional support and informal counseling within the scope of the position.
  • Identify residents who may benefit from additional psychosocial services or community resources and coordinate appropriate referrals.
  • Maintain knowledge of community resources and services available to residents and families.
  • Maintain accurate and organized Social Services Department records and files.
  • Assist with grievances and resident/family concerns and ensure appropriate follow-up and documentation.
  • Maintain confidentiality of resident information and comply with HIPAA requirements.
  • Participate in facility meetings, care conferences, quality improvement activities, and required in-services.
  • Educate and assist staff regarding resident rights, psychosocial needs, communication, and other social-service-related topics as appropriate.
  • Maintain professional and respectful communication with residents, families, visitors, coworkers, and leadership.
  • Immediately report concerns involving resident safety, abuse, neglect, exploitation, or other reportable events in accordance with facility policy and applicable regulations.
  • Follow all facility policies, safety procedures, and applicable federal and state requirements.
  • Perform other duties as assigned to support residents and facility operations.

Position Summary

The Social Services Director is responsible for the day-to-day operation of the Social Services Department and for coordinating social services that support the mental, emotional, and psychosocial well-being of our residents.

This position works closely with residents, families, nursing, administration, therapy, activities, and other members of the interdisciplinary team to identify resident needs, coordinate services, assist with care planning, and support safe transitions and discharges.
