Job summary

Gary and Mary West PACE in San Marcos, CA, is seeking a Social Services MSW to coordinate and support all aspects of the Social Services Department. You will work under the Social Services Supervisor, ensuring care coordination for participants and collaboration with the interdisciplinary team.

Requirements include a High School Diploma or GED, BLS/CPR certification, CA driver's license, and at least one year in geriatric or behavioral health settings.

Qualifications

• High School Diploma or GED required.
• Current Basic Life Support and CPR certification required.
• Current California Driver's License with proof of auto insurance.
• Minimum one year of experience with frail/elderly populations.
• Experience in behavioral health or social services setting preferred.

Responsibilities

• Participate as a supportive member of the interdisciplinary team and track tasks.
• Attend morning IDT meetings and report task status.
• Coordinate MSW schedule for in-person assessments and care conferences.
• Prepare MSW for assessments and care planning meetings.
• Present care plans to participants and obtain signatures for EMR upload.
• Maintain case management records within 72 hours of encounters.
