Job summary

Gary and Mary West PACE seeks a dedicated social worker to manage cases for participants, working with an interdisciplinary team to assess needs and plan care. You will visit homes, conduct intake and family meetings, provide counseling, and advocate for participants’ safety and wellbeing.

Qualifications include a Master’s in social work, a valid driver’s license, and bilingual English/Spanish preferred. This on-site role supports seniors in the North County region with a mission-driven culture.

Qualifications

• Master's degree in social work is required.
• Original valid driver's license with proof of auto insurance.
• Bilingual English/Spanish preferred.

Responsibilities

• Provide direct social work case management services to participants.
• Participate with the interdisciplinary team and conduct assessments.
• Visit participants' homes to assess safety and wellbeing for care planning.
• Coordinate family meetings and intake processes with the IDT.
• Provide crisis intervention and advocacy as needed.
• Maintain confidential case records and ongoing documentation.
