Job summary

Senior Care Partners P.A.C.E. in Battle Creek, MI is hiring a Clinical Care Registered Nurse to provide direct nursing care to participants in both clinic and home settings.

The role requires independent nursing judgment, collaboration with IDT, and management of complex clinical situations. Travel between centers may be required; CPR certification is expected. Employment offers professional development and competitive pay.

Qualifications

• Bachelor’s degree in Nursing preferred or equivalent.
• One year minimum experience with frail/elderly or long-term care.
• Home health/home care experience preferred.
• CPR certification current or willing to obtain.
• Strong communication and confidentiality skills.

Responsibilities

• Routine monitoring of acute and chronic medical conditions following the participants' goals of care.
• Performs wound care and treatments as prescribed in the participant's plan of care.
• Administers medications, immunizations, and other care as prescribed.
• Performs assessments and actively participates in care plans.
• Makes home visits regularly to provide care and treatment as ordered by the PCP.
• May include wound care, lab draws, nurse assessments, and other nursing tasks.
• On-call needs and determines immediate needs for the participant as needed.
• Works beyond normal working hours and on weekends and holidays, as necessary.
• Must complete all required education and employee file requirements on time.
• Documents in the participant’s medical record as required by policy.
• Must care, support, and collaborate with everyone involved in the participant’s care, including staff and the participant’s family.
• Keeps the leader informed of problems or issues.
• Performs other duties as assigned.
