Job summary

Mercyhealth Visiting Nurses Association, Inc. is seeking a Case Manager/Care Coordinator who conducts comprehensive needs assessments in participants' homes or facilities within the Illinois CCP framework.

The role coordinates CCP services, applies for benefits, and ensures timely care planning and follow-up for changing needs. The position requires a social sciences degree, or LPN/RN credentials, plus experience in elderly services and needs assessment.

Qualifications

• Bachelor's degree in social science, social work, or related field.
• One year of experience providing services for the elderly; or LPN/RN.
• One year experience in needs assessment and care planning.

Responsibilities

• Performs comprehensive assessments in participant's home or facility.
• Authorizes CCP services and coordinates referrals.
• Completes Choices for Care assessments for nursing home or supportive living placements.
• Completes Medicaid applications when indicated.
• Documents and follows up on critical events.
• Responds to participant/family inquiries promptly.
