Job summary

Hartford HealthCare is seeking a Resource Coordinator for the Center for Healthy Aging. This role organizes information, programs and activities, connecting seniors and their families to essential services to improve quality of life.

Responsibilities include assisting clients, coordinating referrals, and supporting caregivers with education and program delivery. Experience in case management for older adults and strong communication are essential.

Qualifications

• Bachelor's degree in Human Services or related field is required.
• 3 years' experience in case management or discharge planning with older adults is preferred.
• Proficient in Microsoft Office applications (Word, Excel, Access, PowerPoint).
• Exceptional customer service and communication skills are essential.
• Ability to work independently in a high-volume environment.
• Detail-oriented with strong organizational skills.
• Valid CT driver's license and ability to travel within the community served.

Responsibilities

• Assess and assist seniors and families to identify needs and connect to resources, including home visits.
• Assist in education and training programs for caregivers, elders and family members.
• Acquire and maintain resource materials (brochures, books, tapes, etc.).
• Market the Center to professionals and referral sources within the Hartford HealthCare System and the community.
• Maintain liaisons with seniors, families, support personnel to ensure needs are met.
• Communicate with physicians and other health care providers per HIPAA requirements.
• Build and maintain relationships with hospitals, social service agencies, senior centers, and community groups.
• Process referrals and assist seniors in transitions between levels of care and providers.
