Job summary
Tender Touch Rehab Services is seeking a compassionate Certified Physical Therapist Assistant to join our Healthcare & Medical Services team, providing individualized therapy to seniors in skilled nursing and long-term care settings. This on-site role focuses on mobility, function, safety, and quality of life while documenting progress and collaborating with caregivers and an interdisciplinary team.
You will implement PT treatment plans, perform therapeutic exercises, gait and balance training,
Qualifications
- Certified Physical Therapist Assistant licensed to practice.
- Experience delivering therapy to seniors in skilled nursing or long-term care settings.
- Ability to work collaboratively with an interdisciplinary team.
Responsibilities
- Implement PT treatment plans as prescribed by the physical therapist.
- Perform therapeutic exercises, gait and balance training, and functional activities.
- Document patient progress accurately in the EMR.
- Educate patients and caregivers to support recovery and safety.
- Collaborate with a supportive, patient-centered care team.
