Job summary

Tender Touch Rehab Services is seeking a compassionate Certified Physical Therapist Assistant to join our Healthcare & Medical Services team. This on-site role focuses on providing individualized therapy to seniors in skilled nursing and long-term care settings, implementing PT treatment plans to improve mobility, safety and quality of life.

You will perform therapeutic exercise, gait and balance training, functional activities, and education for patients and caregivers, while documenting care

Qualifications

• Certified PTA licensed to practice in the state.
• Experience in geriatric skilled nursing or long-term care preferred.
• Strong ability to document care accurately in EMR.

Responsibilities

• Implement PT treatment plans to improve mobility, function and safety.
• Conduct therapeutic exercise and gait/balance training with seniors.
• Perform functional activities and caregiver education.
• Maintain accurate EMR documentation and team communication.
