VenVisor Health in Weaverville, NC is hiring a Primary Care NP/PA to deliver on-site care in long-term care facilities. This full-time role offers comprehensive patient care, collaborative practice, and meaningful interactions with residents.

The position requires an unrestricted NP/PA license for NC, at least 6 months in long-term care, strong communication and clinical skills, and the ability to work in a team to develop care plans and provide patient education.

Formación

• Active and unrestricted NP/PA license in NC.
• Minimum 6 months of experience in long-term care or a skilled nursing facility.
• State-specific license in good standing in North Carolina.
• Strong communication, clinical, and critical thinking skills.
• Prolonged standing/walking; lifting up to 20 lbs; computer use.

Responsabilidades

• Provide primary care services to residents in skilled nursing and long-term care facilities.
• Perform physical exams and medical history assessments.
• Diagnose and treat chronic and acute health conditions.
• Order and interpret diagnostic tests and lab results.
• Develop and adjust individualized care plans.
• Prescribe medications and monitor treatment effectiveness.
• Collaborate with facility staff and interdisciplinary teams.
• Educate patients and families about treatment options and care goals.
• Ensure a safe, respectful, and supportive environment for residents.
