Job summary

VenVisor Health is seeking a Primary Care Nurse Practitioner/Physician’s Assistant to provide on-site care in long-term care facilities in Concord, NC. This full-time role requires an active NP or PA license in NC, with 6+ months in LTC.

You will perform exams, diagnose conditions, manage care plans, and collaborate with interdisciplinary teams to ensure high-quality elder care. A competitive six-figure compensation is offered.

Qualifications

• Active and unrestricted NP/PA license.
• State-specific license in good standing in North Carolina.
• Minimum 6 months of experience in long-term care or a skilled nursing facility.

Responsibilities

• Provide primary care services to residents in skilled nursing and long-term care facilities.
• Perform physical exams and medical history assessments.
• Diagnose and treat chronic and acute health conditions.
• Order and interpret diagnostic tests and lab results.
• Develop and adjust individualized care plans.
• Prescribe medications and monitor treatment effectiveness.
• Collaborate with facility staff and interdisciplinary teams.
• Educate patients and families about treatment options and care goals.
