Job summary

VenVisor Health is seeking a Primary Care NP/PA to deliver on-site care to residents in skilled nursing and long-term care facilities in Durham, NC. This full-time role emphasizes meaningful patient relationships and collaborative care.

You will perform exams, manage chronic and acute conditions, order tests, develop care plans, and educate residents and families while collaborating with interdisciplinary teams. Strong communication and elder-care passion are essential.

Qualifications

• Active and unrestricted NC NP/PA license.
• License in good standing in North Carolina.
• Minimum 6 months experience in long-term care or skilled nursing facilities.
• Strong communication, clinical, and critical thinking skills; elder-care focus.

Responsibilities

• Provide primary care services to residents in skilled nursing and long-term care facilities.
• Perform physical exams and medical history assessments.
• Diagnose and treat chronic and acute health conditions.
• Order and interpret diagnostic tests and lab results.
• Develop and adjust individualized care plans.
• Prescribe medications and monitor treatment effectiveness.
• Collaborate with facility staff and interdisciplinary teams.
• Educate patients and families about treatment options and care goals.
• Ensure a safe, respectful, and supportive environment for residents.
