Responsibilities

• Coordinate patient care plans in collaboration with healthcare providers, patients, and families to ensure comprehensive and individualized care
• Gather and review patient medical records, treatment plans, and laboratory results to identify potential issues and develop effective care plans
• Develop relationships with healthcare providers, including physicians, therapists, and nurse practitioners, to ensure seamless communication and coordination of patient care
• Monitor patient progress, identify areas for improvement, and adjust care plans accordingly
• Maintain accurate and detailed records of patient care, including treatment plans, progress notes, and incident reports
