Job summary

Visalia Post Acute is seeking a skilled Physical Therapist (PT) to join our rehabilitation team in Visalia, CA. This full-time role focuses on evaluating and treating residents in a skilled nursing and short-term rehab setting, with leadership responsibilities over PTAs and staff.

The PT will develop individualized plans of care, coordinate with physicians, and ensure timely documentation while assisting in discharge planning and caregiver education.

Qualifications

• Bachelor’s degree in Physical Therapy required.
• Master’s or Doctorate in Physical Therapy preferred.
• Casamba and PointClickCare experience helpful.

Responsibilities

• Evaluate residents within 24 hours of referral and document.
• Develop individualized treatment plans; obtain physician approvals.
• Supervise PTAs, aides, and students per state act.
• Communicate progress to rehab teams and families.
• Complete evaluations, notes, and discharge documentation.
• Educate residents and families on mobility and safety.
• Participate in discharge planning and DME coordination.
