Qualifications

• Licensed Physical Therapist (PT) or license eligible in Ohio.
• Graduate of an accredited Physical Therapist program.

Responsibilities

• Evaluate patients and provide personalized physical therapy treatments that help improve mobility and independence.
• Develop and implement patient-centered treatment plans for long-term rehabilitation goals.
• Deliver hands-on therapy interventions to support recovery and endurance in older adults.
• Help patients restore functional mobility for daily activities with greater confidence and independence.
• Partner with interdisciplinary teams to drive meaningful outcomes and continuity of care.
• Track patient progress and document accurately, adapting plans as needed.
