Job summary

Sage Health in the United States is seeking a Primary Care Physician to lead a team-based care model focused on seniors under a value-based system. The role emphasizes high quality care in a concierge-style setting, with coordinated services and analytics-supported decisions.

The PCP will see fewer patients, spend more time with each patient, and collaborate with specialists and hospitals to achieve better outcomes within a Medicare Advantage framework.

Qualifications

• MD or DO degree from an accredited program
• Current and unrestricted medical license in state of practice or ability to obtain one
• Board certification or eligibility in Family/Internal/Geriatric medicine preferred
• Active DEA license and Medicaid provider number

Responsibilities

• Evaluate and treat center patients per standards of care
• Coordinate patient services including referrals and hospital/SNF coordination
• Participate in daily huddles with the care team to share medical expertise
• Lead with a professional and approachable demeanor in the practice
• Complete medical record documentation daily with a quality-based coder
• Review quality data and trends with Lead Physician
• Balance direct patient care with administrative duties (charting, meetings)
