Qualifications

• High school diploma or equivalent required.
• 2-4 years work experience; healthcare or senior care preferred.
• Experience working with seniors or caregivers beneficial.
• Advanced computer skills including CRM software and Microsoft Office.

Responsibilities

• Respond to information and referral inquiries by phone, email or written communication.
• Build rapport with callers to gather information and identify referrals.
• Schedule visits and coordinate with enrollment coordinators for referrals.
• Document outcomes and data in CRM with accuracy and completeness.
• Develop and maintain referral relationships with health and human service professionals.
• Travel locally to senior housing communities as needed.
• Support marketing communications and team collaboration to improve enrollment.
• Provide administrative support to department managers as required.
