Oasis Therapy Services in Evansville, Wisconsin, seeks an Occupational Therapist (OT) to provide one-on-one rehabilitation in an assisted living setting. PRN opportunities with Net Health EMR support are available.

Responsibilities include evaluating residents, creating individualized plans, and educating residents and caregivers on exercises and equipment. Licensure, CPR, and strong communication are required.

Formación

• Active Wisconsin OT license required.
• Graduate of an accredited OT program.
• Current CPR certification.
• Strong communication and documentation skills.
• Geriatric or rehab experience preferred.

Responsabilidades

• Evaluate residents and develop individualized treatment plans.
• Provide therapy focused on ADLs, mobility, safety, and independence.
• Monitor progress and adjust treatment plans as needed.
• Educate residents and caregivers on exercises, adaptive techniques, and equipment.
• Complete Net Health documentation and collaborate with the care team.
