Job summary

UnitedHealth Group is seeking an Advanced Practice Clinician (NP/PA) to deliver integrated care within the Senior Community Care program. You will coordinate clinical care for high-risk members across settings, including nursing homes and senior living facilities, while avoiding on-call, weekend, and holiday shifts.

You will work with a multidisciplinary team to conduct assessments, plan advanced care, and educate patients and families.

Qualifications

• Active ANCC or AANP certification in Family, Adult, GerIatric, or Adult-Gerontology Primary Care.
• Current NCCPA certification for PAs and state licensure.
• Active, unrestricted licensure in the state of practice with no disciplinary actions.
• Driver's license and reliable transportation.
• DEA licensure/prescriptive authority or ability to obtain post-hire.
• Ability to obtain a collaborative practice agreement if required.
• Ability to navigate varied environments and duties.

Responsibilities

• Conduct comprehensive assessments including medical history, vitals, exam.
• Provide care during short-term sub-acute rehabilitation episodes.
• Ensure accurate documentation with ICD-10 coding.
• Collaborate with interdisciplinary care teams.
• Engage in advanced illness and end-of-life conversations.
• Counsel patients and caregivers and provide education.
