Infinite Medical P.C. is seeking a Primary Care Nurse Practitioner/Physician’s Assistant to provide on-site care in long-term care settings in Fall River, MA. This full-time role emphasizes meaningful patient relationships and collaborative care.

The provider will work within a nationwide network, delivering proactive care to residents in skilled nursing facilities, with a compensation range of $125,000 to $175,000 per year.

Qualifications

• Active and unrestricted NP/PA/Physician license.
• State-specific license in good standing in Massachusetts.
• Minimum 6 months of experience in long-term care or a skilled nursing facility.

Responsibilities

• Provide primary care services to residents in skilled nursing and long-term care facilities.
• Perform physical exams and medical history assessments.
• Diagnose and treat chronic and acute health conditions.
• Order and interpret diagnostic tests and lab results.
• Develop and adjust individualized care plans.
• Prescribe medications and monitor treatment effectiveness.
• Collaborate with facility staff and interdisciplinary teams.
• Educate patients and families about treatment options and care goals.
• Ensure a safe, respectful, and supportive environment for residents.
