Infinite Medical P.C. is hiring a Primary Care Nurse Practitioner/Physician’s Assistant to provide on-site primary care in long-term care facilities in Randolph, Vermont.

This full-time role offers a competitive salary and the opportunity to impact elder care through collaborative, patient-centered practice. The position requires an active NP/PA license with VT state licensure in good standing and at least 6 months’ LTC experience.

Qualifications

• Active and unrestricted NP/PA/Physician license.
• VT state license in good standing.
• Minimum 6 months of long-term care or skilled nursing facility experience.
• Excellent communication, clinical, and problem-solving skills.
• Passion for elder care and person-centered healthcare.

Responsabilités

• Provide primary care to residents in skilled nursing and long-term care facilities.
• Perform physical exams and collect medical histories.
• Diagnose and treat chronic and acute conditions.
• Order and interpret labs and diagnostic tests.
• Develop and adjust individualized care plans.
• Prescribe medications and monitor effectiveness.
• Collaborate with facility staff and interdisciplinary teams.
• Educate patients and families about treatment options and goals.
• Maintain a safe, respectful care environment.
