Job summary

Infinite Medical P.C. is seeking a Primary Care Nurse Practitioner or Physician Assistant to provide on-site care in long-term care facilities in Holyoke, MA.

This full-time role offers a competitive total annual compensation and a collaborative care environment with an emphasis on elder patient relationships. Qualified candidates hold an active NP/PA license in MA, have a minimum of six months in long-term care, and are committed to high-quality proactive care for residents.

Qualifications

• Active and unrestricted NP/PA license
• Massachusetts license in good standing
• Minimum 6 months of experience in long-term care
• Strong communication, clinical, and critical thinking skills
• Passion for elder care and person-centered healthcare
• Ability to stand/walk for extended periods and assist with lifting up to 20 lbs
• Competitive salary range stated in ad: $150,000-$200,000 per year

Responsibilities

• Provide primary care services to residents in skilled nursing and long-term care facilities
• Perform physical exams and medical history assessments
• Diagnose and treat chronic and acute health conditions
• Order and interpret diagnostic tests and lab results
• Develop and adjust individualized care plans
• Prescribe medications and monitor treatment effectiveness
• Collaborate with facility staff and interdisciplinary teams
• Educate patients and families about treatment options and care goals
• Ensure a safe, respectful, and supportive environment for residents
