Infinite Medical P.C. is hiring a Primary Care Nurse Practitioner/Physician’s Assistant to provide on-site care for residents in long-term care facilities in Swansea, MA. This full-time role offers a meaningful patient relationship within a collaborative care environment.

The position requires active MA-licensed NP/PA/MD, at least 6 months in long-term care, strong communication, and a passion for elder care. Competitive compensation is offered.

Qualifications

• Active and unrestricted NP/PA/Physician license in good standing.
• State license in Massachusetts in good standing.
• Minimum 6 months of experience in long-term care or skilled nursing facilities.
• Strong communication, clinical & critical thinking skills.
• Passion for elder care and patient-centered healthcare.
• Competitive salary per annum: $150,000–$200,000.

Responsibilities

• Provide primary care services to residents in skilled nursing & long-term care facilities.
• Perform physical exams and comprehensive medical histories.
• Diagnose and treat chronic and acute conditions.
• Order and interpret diagnostic tests and lab results.
• Develop and adjust individualized care plans.
• Prescribe medications and monitor treatment effectiveness.
• Collaborate with facility staff and interdisciplinary teams.
• Educate patients and families about treatment options and goals.
• Maintain a safe, respectful environment for residents.
