Job summary

VenVisor Health is seeking a Primary Care Nurse Practitioner/Physician’s Assistant to provide on-site primary care to residents in skilled nursing and long-term care settings in Concord, NC. This full-time position emphasizes meaningful patient relationships and collaborative, team-based care.

The ideal candidate holds an active NP/PA/MD license, NC state license in good standing, and has at least 6 months of LTC experience.

Qualifications

• Active, unrestricted NP/PA license in good standing.
• Minimum 6 months of experience in long-term care or skilled nursing facilities.
• Excellent communication, clinical, and critical-thinking skills.
• Patient-centered elder care approach.

Responsibilities

• Provide primary care in skilled nursing and long-term care facilities.
• Perform physical exams and medical history assessments.
• Diagnose and treat chronic and acute health conditions.
• Prescribe medications and monitor treatment effectiveness.
• Collaborate with facility staff and interdisciplinary teams.
• Educate residents and families about treatment options and goals.
