Job summary

Curana Health, Inc. is seeking a compassionate Nurse Practitioner or Physician Assistant to deliver high-quality primary care in senior living facilities. You will manage preventive care, chronic conditions, and coordinate with other providers to ensure seamless patient care.

In this role, you will work closely with facility staff and families, conduct rounds, educate patients, and maintain thorough EMR documentation while meeting regulatory standards. Travel to assigned facilities is required.

Qualifications

• Valid nurse practitioner or physician assistant license in state of operations and current board certification.
• Masters degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem-solving skills.
• Excellent written and oral communication skills.
• Punctuality, organizational skills, and ability to function within a team.

Responsibilities

• Perform comprehensive assessments and develop treatment plans for residents.
• Conduct routine rounds and monitor residents' conditions.
• Coordinate care with facility staff, families, and other providers.
• Educate residents and families on wellness and chronic disease management.
• Maintain accurate EMR documentation and ensure regulatory compliance.
• Participate in quality improvement initiatives to reduce readmissions.
