Curana Health, Inc. seeks nurse practitioners and physician assistants to deliver high-quality primary care directly in senior living communities.

You will conduct full health assessments, develop individualized care plans, and manage chronic conditions with autonomy and support from experienced physicians and leaders. You will collaborate across care teams, leverage advanced technology and predictive analytics, and travel to multiple facilities to reach residents where they live.

Competenze

• Valid nurse practitioner or physician assistant license
• Masters degree in nursing or PA studies
• Strong leadership and communication skills
• Ability to work well in a team and stay organized

Mansioni

• Deliver high-quality primary care to residents in skilled nursing and living communities.
• Build meaningful patient relationships and provide preventative care and care management.
• Collaborate with physicians, staff, and families to coordinate comprehensive care plans.
