Curana Health is seeking a Nurse Practitioner or Physician Assistant to lead primary care services across our senior living communities. You will deliver proactive, relationship-based care directly where residents live, with autonomy and support from a national team of clinicians and leaders.

Qualifikationen

• Valid NP/PA license in the state of operations.
• Master's degree in Nursing or Physician Assistant Studies.
• Excellent written and oral communication skills; strong leadership and teamwork abilities.

Aufgaben

• Provide comprehensive, primary care to residents in skilled nursing, assisted living, and independent living communities.
• Diagnose and treat acute and chronic conditions with emphasis on preventative care and long-term wellness.
• Maintain EMR documentation accurately and timely; ensure regulatory and organizational compliance.
• Collaborate with physicians, clinical staff, and facility leadership to coordinate care plans.
• Build and sustain meaningful patient relationships and educate residents and caregivers on disease management.
