Curana Health is seeking a compassionate Nurse Practitioner or Physician Assistant to deliver high-quality primary care in skilled nursing and senior living facilities in Louisiana. You will provide direct patient care, focusing on preventive care, chronic disease management, and coordinating care with other healthcare providers.

Competenze

• Must hold valid NP or PA license in state of operation
• Master’s degree in nursing or PA studies
• Strong leadership, interpersonal, and problem-solving skills
• Excellent written and oral communication skills

Mansioni

• Provide direct patient care and comprehensive assessments in SNF and senior living facilities
• Perform routine rounds, monitor residents, and adjust treatment plans as needed
• Coordinate care with facility staff, families, and other healthcare providers
• Educate residents and families on disease prevention and wellness
• Maintain accurate EMR documentation and ensure regulatory compliance
• Participate in quality improvement initiatives to enhance care and reduce readmissions
