Qualifications

• Valid nurse practitioner or physician assistant license in your state of operation and current board certification.
• Master's degree in nursing or physician assistant studies.
• Strong leadership, interpersonal, and problem-solving skills preferred.
• Excellent written and oral communication skills.
• Punctuality, organizational skills, and ability to work well in a team.

Responsibilities

• Deliver high-quality primary care by performing comprehensive health assessments and creating individualized treatment plans.
• Diagnose, treat, and manage acute and chronic conditions with focus on prevention and long-term wellness.
• Provide ongoing care management to improve quality of life and residents' independence.
• Conduct facility rounds and build strong relationships with residents, families, and care teams.
• Coordinate care with physicians and other clinical staff and serve as a clinical resource within the facility.
