Job summary

AllCare.ai is seeking Primary Care NPs to deliver on-site care to Assisted Living/RCFE residents across Stockton, Modesto, and Amador. You will perform routine and urgent assessments, manage chronic diseases, and coordinate with facility staff, specialists, and families.

The role features a structured weekday schedule: three on-site days with clustered routes, plus two remote days for documentation and administrative tasks. W-2 with full benefits included.

Qualifications

• Active CA NP and RN licenses, furnishing license.
• NPI, DEA, and BLS certifications.
• Geriatrics or facility-based experience preferred; EMR proficiency.

Responsibilities

• Perform routine and urgent assessments; manage chronic diseases.
• Post-discharge follow-ups, med reconciliation, labs/diagnostics.
• Coordinate with facility staff, specialists, families/POAs.
• Document same-day care and ensure CMS/regulatory compliance.
