Qualifications

• Licensed as a Nurse Practitioner or Physician Assistant with active board certification.
• Strong communication and collaboration skills across care teams.
• Experience leading clinical initiatives and improving patient outcomes.

Responsibilities

• Provide high-quality primary care in senior living communities and skilled nursing facilities.
• Develop individualized care plans and manage acute and chronic conditions.
• Maintain thorough EMR documentation and ensure regulatory compliance.
• Collaborate with physicians, staff, and families to coordinate care.
