Qualifications

• Licensed as a nurse practitioner or physician assistant in the operating state and board-certified.
• Master’s degree in nursing or physician assistant studies.
• Strong leadership, interpersonal and problem‑solving skills; excellent written and verbal communication.

Responsibilities

• Perform comprehensive patient assessments and develop treatment plans for residents.
• Conduct routine rounds, adjust treatment plans, and address health concerns.
• Coordinate care with facility staff, families, and other healthcare providers.
• Educate residents and families on disease prevention and chronic illness management.
• Maintain accurate EMR documentation and ensure regulatory compliance.
• Participate in quality improvement initiatives to reduce readmissions and improve outcomes.
