Candidate Requirements
  • Reside in Florida, specifically in Orange County, Seminole County, Osceola County, Brevard County, or surrounding areas.
  • Must have four or more years of Pediatric experience and LPN licensure.
Position Purpose

Assess, plan, implement, and coordinate care management activities based on member needs to provide quality, cost-effective healthcare outcomes. Develop or contribute to personalized care plans and educate members and their families on available services and benefits to improve healthcare access and quality through advocacy and coordination.

Key Responsibilities
  • Develop or contribute to ongoing care plans and identify providers, specialists, and community resources.
  • Coordinate between members, families, caregivers, and care teams to ensure timely access to services.
  • Monitor progress towards care goals and collaborate with healthcare providers for plan revisions.
  • Provide psychosocial support and resource guidance to members and caregivers.
  • Identify barriers to care and implement appropriate interventions.
  • Review referral information to assist in developing care plans for members with higher needs.
  • Collaborate with healthcare partners to ensure member needs are met and revise plans as necessary.
  • Maintain all member information and activities in compliance with regulations.
  • May conduct on-site assessments and provide education on procedures, options, and benefits.
  • Serve as a Subject Matter Expert and support team training or onboarding.
  • Perform other duties as assigned.
Education and Experience

Bachelor’s degree with 4-6 years of related experience; nursing graduates must have an accredited nursing degree. Equivalent experience may be considered.

Licenses and Certifications
  • Current state clinical license preferred.
  • For CMS Florida licensed practical nurses: minimum of four years pediatric experience, or a Master’s in social work with at least one year of pediatric care experience. Travel may be required.
Compensation and Benefits

Pay range: $26.50 - $47.59 per hour. Benefits include health insurance, 401K, stock plans, tuition reimbursement, paid time off, and flexible work arrangements. Compensation depends on skills, experience, and other factors.
