Qualifications

• Candidate must reside in Florida, in Pasco County, Pinellas County, or surrounding areas.
• Must have four or more years of Pediatric experience and LPN licensure.

Position Purpose

Assesses, plans, implements, and coordinates care management activities based on member needs to provide quality, cost-effective healthcare outcomes. Develops or contributes to the development of a personalized care plan/service plan for members and educates members and their families/caregivers on available services and benefits to improve healthcare access and quality care through advocacy and coordination.

Responsibilities

1. Develops or contributes to ongoing care plans and identifies providers, specialists, and community resources needed for care.
2. Coordinates between members, families/caregivers, and care providers to ensure timely access to services.
3. Monitors progress towards care plan goals and collaborates with healthcare providers for revisions as needed.
4. Provides psychosocial and resource support to members, caregivers, and care managers.
5. Identifies barriers for care coordination and intervenes for members with higher care needs.
6. Reviews referrals and assessments to develop appropriate care plans.
7. Collaborates with healthcare partners to meet member needs and revise care plans accordingly.
8. Maintains all member information and activities for compliance.
9. May perform on-site assessments and provide education on procedures and benefits.
10. Serves as a Subject Matter Expert and supports new staff.
11. Performs other duties as assigned, complying with policies and standards.

Education and Experience

Bachelor’s degree and 4-6 years of related experience, or equivalent experience. Clinical licensure (LPN or RN) preferred.

Licenses and Certifications

• Current state clinical license preferred.
• For CMS Florida LPN, minimum four years pediatric experience; or a Master’s in social work with at least one year of pediatric care experience.

Additional Details

Pay Range: $26.50 - $47.59 per hour.
