This is a remote role. Candidates must reside in Florida. Must have four years of Pediatric experience and LPN licensure.

Position Purpose

Assesses, plans, implements, and coordinates care management activities based on member needs to provide quality, cost-effective healthcare outcomes. Develops or contributes to the development of a personalized care plan/service plan for members and educates members and their families/caregivers on available services and benefits to improve healthcare access and quality through advocacy and coordination.

Responsibilities
  1. Develops or contributes to ongoing care plans and identifies providers, specialists, and community resources needed for care.
  2. Coordinates and manages communication between members, caregivers, and care providers to ensure timely access to services.
  3. Monitors progress towards care plan goals and collaborates with healthcare providers for revisions or addressing member needs.
  4. Provides psychosocial and resource support to members/caregivers, assisting access to local resources based on assessments.
  5. Identifies barriers to care and intervenes appropriately for members with higher care needs.
  6. Reviews referrals and assessments to develop or assist in developing care plans for high-need members.
  7. Collaborates with healthcare partners to ensure member needs are met and revise care plans as necessary.
  8. Maintains all member information and activities to ensure compliance with regulations.
  9. May perform on-site visits and provide education to care managers and members.
  10. Serves as SME or preceptor for new staff.
  11. Performs other duties as assigned and complies with policies.
Requirements
  • Bachelor’s degree with 4-6 years related experience or equivalent clinical experience.
  • Current state clinical license preferred.
  • For LPNs: minimum of four years pediatric experience; may require up to 80% local travel.
Additional Information

Pay Range: $26.50 - $47.59 per hour.
