LMK Recruiting is seeking dedicated individuals for the role of Clinical Care Manager to ensure our clients receive high-quality support in home, medical, and facility settings. The position emphasizes comprehensive care planning, advocacy, and collaborative teamwork across a heavily field-based role in the Virginia and DC metro area.

Qualifications

• Master's degree in a related field or a bachelor's degree with at least 4 years of relevant experience in gerontology, mental health, home care, or similar.
• Available 9am to 6pm Monday-Friday and some weekends.
• Dependable, adaptable, and accountable, with a strong commitment to providing exceptional care.
• Must be able to lift and maneuver up to 50 pounds.
• Must be comfortable and willing to drive regularly around the Virginia and DC Metro area.

Responsibilities

• Comprehensive Care Management: Oversee and coordinate tailored care plans, ensuring clients receive appropriate and timely services.
• Client Advocacy: Serve as a strong advocate for clients, ensuring needs are met in home, medical and care facility settings.
• Support and Collaboration: Work closely with team to address clients' needs and enhance collective knowledge.
• On-Call Availability: Participate in after-hours or weekend on-call rotations as needed.
• Field Work and Documentation: Update family and care team members; 90% field work; 10% office tasks.
