Job summary

Arosa in Houston, TX is seeking a Care Management & Outreach professional to work directly with clients and their families, assess needs across physical, emotional, social, and spiritual domains, and develop comprehensive care plans.

You will engage with local professionals and committees to educate, build relationships, and create solutions, helping aging clients access in-home care and community-based services, with emphasis on compassionate, team-based service.

Qualifications

• Bachelor’s Degree required; Master’s preferred in healthcare-related field.
• Ability to interface professionally with client support, including family, physicians, attorneys.
• Ability to work in a team environment.
• Bachelor’s or Master’s in Social Work, Nursing, Gerontology, Psychology, Health and Human Services, Occupational Therapy, Licensure in nursing (LVN/LPN or RN), or related fields.
• Minimum of two years experience in care management; aging population preferred.
• Excellent written and verbal communication skills.
• PC literacy; valid driver's license and current auto insurance.

Responsibilities

• Represent Arosa in the community; education and outreach to professionals; meetings with committees/boards.
