Job summary

Arosa is a national in-home care and care management provider seeking a Care Management & Professional Outreach professional in Massachusetts. You will work with clients and families to assess needs, develop care plans, and ensure stability and peace of mind, coordinating services across teams and providers.

You will also represent Arosa in the community, educate professionals, attend meetings, and build solutions with physicians, attorneys, and other senior service providers.

Qualifications

• Bachelor’s degree required; Master’s degree in healthcare-related field preferred.
• Ability to interface effectively with client support, including family, physicians, attorneys, etc.
• Ability to work in a team environment.
• Licensure or degree in Social Work, Nursing, Gerontology, Psychology, Health and Human Services, or related fields.
• Minimum of two years care management experience preferred; substitutions possible.

Responsibilities

• Work directly with clients and families to assess needs and develop care plans.
• Be our face in the community: educate professionals, join local committees and coordinate meetings.
• Collaborate with professional service providers to ensure integrated home care and care management.
• Maintain clear, professional communication and documentation.
