Arosa Chicagoland seeks a Care Management & Outreach professional to support clients and families with in-home care planning and community-based services. You will assess needs across physical, emotional, and social domains and develop plans to enhance stability and peace of mind.

You will engage with local committees and professional networks to educate on care options, build partnerships, and help professionals connect clients with appropriate services in Illinois.

Qualifications

• Bachelor’s degree required; Master’s in healthcare preferred.
• Excellent written and verbal communication skills required.
• Ability to interface professionally with clients, families, physicians, attorneys, etc.

Responsabilités

• Work directly with clients and families to assess needs and develop care plans.
• Engage with local committees, task forces, and professional groups to educate and coordinate services.
