Job summary

Premier Care Management of Georgia, LLC is seeking a commission-based Independent Contractor RN, Geriatric Care Manager to deliver high-quality assessments and care plans for seniors. You will help families navigate care options with compassion and professionalism.

Responsibilities include engaging clients, conducting home visits, completing intake, developing goal-oriented care plans, coordinating resources, and maintaining daily charting.

Qualifications

• Active Georgia Registered Nurse license is required.
• Bachelor's Degree in Nursing preferred.
• Certification in Care Management or Gerontology preferred.
• Minimum two years in care management or clinical case management, with elderly populations.
• Proven track record in care management and collaborating with families.
• Familiarity with community health resources and service delivery systems.
• Strong communication and relationship-building with seniors and families.
• Analytical, self-disciplined, and able to work independently or in a team.
• Excellent verbal and written communication skills.
• Basic computer literacy for charting and planning.
• Valid driver's license and access to a vehicle.
• This is a commission-based Independent Contractor position (1099).Set your own schedule; typical 32–40 hours per week.
• Willingness to collaborate with a growing team and participate in ongoing training.

Responsibilities

• Represent PremierCMGA to prospective clients and families to engage care management services.
• Work within family systems to meet client needs and enable realistic goals.
• Respond promptly, arrange home visits, and guide intake processes.
• Perform comprehensive home assessments and develop goal-oriented care plans.
• Provide personalized care management based on assessments and company protocols.
• Adjust care levels based on client needs and family preferences.
• Offer coaching, education, and advocacy as required.
• Build strong relationships with clients, families, referral sources and community resources.
• Utilize the online record system for daily charting and routine updates.
• Keep care plans current as client needs change; ensure proper billing procedures.
