Job summary

Devoted Health is seeking a Senior Manager for Transitions of Care (TOC) to lead a frontline team of 60–70 RNs and clinicians. You will partner with TOC leaders to drive high-quality post-discharge care and reduce readmissions, while modeling servant leadership and frontline care excellence.

In this role you will coach managers, develop workflows, and shape program strategy to scale care delivery across a virtual/interdisciplinary system, aligned with our family-centered mission.

Qualifications

• 5+ years leading clinical teams in transitional care or care management settings.
• 3+ years coaching and developing other leaders/managers.
• Active RN license in good standing, and/or BSN or related degree; master's preferred.
• Experience in post-acute medicine, nursing and case management, geriatrics.

Responsibilities

• Transformational Clinician Leadership: build and inspire a high-performing team of frontline nurses, coordinators, educators & nurse leaders.
• Coaching & Development: lead hiring, training, workflow improvements, and performance management for frontline teams.
• Results-Driven Performance Management: track KPIs to reduce avoidable admissions & readmissions; provide coaching & feedback.
• Servant Leadership: perform frontline patient care 10–20% of time to stay connected with operations.
• Thought Partnership: align with clinical & operations leads to optimize outcomes for patients and teams.
