• Leadership experience in clinical care coordination or case management.
• Strong knowledge of care coordination across continuum of care and transitions.

Responsibilities

• Lead and supervise Clinical Care Coordinators and coordinate across ambulatory, ED, post-acute and community resources.
• Identify high-risk patients and develop individualized care plans to address clinical and social needs.
• Assess barriers to care and connect patients with resources to support adherence and engagement.
• Serve as clinical/operational resource on care coordination and population health management.
• Partner with leaders to support value-based care initiatives and budget planning.
• Ensure regulatory, accreditation, payer, and organizational requirements are met.
• Provide direct care coordination support as needed to maintain service delivery.
• Oversee department operations, performance, and staff development.
• Participate in recruiting, coaching, and performance evaluation of team members.
