Job summary

Choice in Aging seeks a dedicated Care Professional to join our team, under the supervision of the Activity Coordinator and Program Director. You will lead and assist with program activities and support frail elders and adults with disabilities to maintain dignity and independence in the community.

You will document daily interactions and wellbeing, provide safety and emotional support, assist with daily living activities, and collaborate with staff to deliver compassionate, person-centered care

Qualifications

• Must be able to communicate clearly both verbally and in written English.
• Must be able to work as a team member.
• Ability to provide safety supervision and emotional support.
• Ability to complete client documentation and logs.

Responsibilities

• Lead, engage and assist with program activities, encouraging client participation by recognition, respect and trust.
• Provide care which respects and values the individuality of each participant.
• Maintain personhood and promote self-worth, agency and social confidence.
• Build one-on-one relationships with participants based on respect.
• Understand participant likes, interests and background to tailor care.
• Interact with participants through constructive conversation and companionship.
• Assist with activities of daily living (toileting, feeding, ambulating, transferring, grooming, hygiene).
• Provide safety supervision and emotional support; manage behaviors when necessary.
• Participate in maintaining a clean program area and perform assigned cleaning duties.
• Assist with food preparation, serving meals, refreshments and cleanup.
• Complete client documentation including progress narratives and logs.
• Maintain confidentiality in all matters of client care.
